Even if your organization is not providing the call type file, please complete the contact and agencies-served information below so we can provide you with WADEPS information.

For the call type information:

* Excel or CSV format required.
* Include your PSAP name in the file name.
* Header row for each column — use underscores instead of spaces.
* Minimum three columns requested:

1. Call or Dispatch Code
2. Primary Description or Nature
3. Secondary Description or detail, if available

If you would like individual agencies to provide the call type information to WADEPS, refer them to the agency version of this form.

General Data Use Agreement (DUA)(Required)

The general DUA between WADEPS and law enforcement agencies governs how data provided to the system is shared, used, and made available to the public, and ensures submitted data mirrors data held independently by participating agencies and their respective dispatch centers.

The DUA includes an optional rider indicating an agency authorizes their PSAP to share data with WADEPS.

Do you anticipate your organization will require formal authorization from all agencies you provide services for?

Yes, we expect all agencies to provide the WADEPS release.
